Objective
This guide outlines the steps required to clear a student for participation in CYBHI, ensuring all necessary consents and information are collected and documented.
Key Steps
Step 1: Access Student Profile 0:38
- Log into the system and navigate to the student's profile.
- Select a student profile that needs to be cleared (any red or yellow student).
- Scroll down to the 'Student Clearance' section located at the bottom left of the profile.
Step 2: Collect Consent to Treat 1:01
- Click on the status of the 'Consent to Treat' section.
- Confirm if the student is self-consenting or not:
- If self-consenting, check the first box. This box allows you to attest that the self-consenting minor is mature enough to proceed.
- If you're not seeking minor consent, or if the minor doesn't meet the maturity requirements, check the second box.
- Sign and hit 'Submit'.
- If the student is not self-consenting, print the consent form or upload a signed paper form. If the student is self-consenting, they will have the option of also signing electronically.
Step 3: Upload Consent Form 2:08
- To upload a paper form, click on 'Upload Paper Form'.
- Browse your device to select the file or take a photo of the signed consent.
- Check the attestation box to certify completion.
- Enter your name to sign and automatically capture the date.
- Click 'Create' to finalize the upload.
Step 4: Verify Consent Status 3:16
- Confirm that the status of the 'Consent to Treat' has changed from red to green.
Step 5: Collect Insurance Information 3:33
- Click on the status of the 'Insurance Information' section.
- Click on 'Add' to enter insurance details:
- Indicate if it is primary insurance.
- Fill out required fields: Insurance company name, Member ID, Cardholder's first and last name.
- Optionally, upload a photo of the insurance card. This helps with verification and claim submission but is not a requirement.
Step 6: Submit Insurance Information 4:52
- After entering the required information, submit the insurance details.
- Refresh the profile to ensure the insurance information is displayed.
Step 7: Collect Consent to Bill 5:17
- Click on the status of the 'Consent to Bill' section.
- If the student is self-consenting, check the first box to confirm maturity attestation.
- Sign and submit the consent.
Step 8: Complete Electronic Consent Form 6:00
- Choose to complete an electronic version of the consent form.
- Have the student sign the electronic form, which will include their name and date.
- Click 'Create' to add this consent to the student's profile.
Cautionary Notes
- You won't be able to document a service for a student without at least a consent to treat. (However, there is a bypass available for crisis situations).
- You won't be able to submit a claim for a student until all three items are collected and the student is "green".
